Helping you find and access the resources you need.
Managing your mental health can be difficult enough. Trying to navigate housing programs, health care, community services, transportation, benefits, and other resources at the same time can quickly become overwhelming.
Mental health case management provides practical support for people who need help finding, accessing, and coordinating services in their community. At Calm Waters Counseling, our case managers work with eligible clients to understand what they need, identify available resources, and help them take the next steps toward greater stability.
What Is Mental Health Case Management?
You don't have to navigate every system on your own.
A case manager helps connect you with services and resources that can support your mental health and everyday needs. This is different from counseling.
Your counselor focuses primarily on your mental health treatment. A case manager focuses on helping you navigate the practical circumstances that may be affecting your ability to move forward.
That may mean helping you understand what programs are available, identifying organizations that could help, coordinating with other providers, or helping you take the steps necessary to access services. The goal is simple: help you get connected to the support you need.
Case management may help connect you with:
- Housing and basic-needs assistance
- Medical and mental health providers
- Transportation and financial resources
- Family, youth, and domestic violence services
- Employment and vocational programs
- Community and social service agencies
We help you understand your options and connect with organizations and programs that may be able to help.
The specific resources available will depend on your needs, circumstances, eligibility, and programs available in the community.
What Does a Case Manager Do?
Your case manager helps turn needs into next steps. Case management is a collaborative process. Your case manager will work with you to understand what is happening, identify your priorities, and determine what resources may be available.
Assessing Your Needs
Your case manager will talk with you about the challenges you're facing and identify areas where additional support may help.
Finding Resources
Instead of expecting you to know which agencies or programs to call, your case manager can help identify appropriate community resources.
Connecting You With Services
Your case manager can help you understand how to contact programs, what information may be required, and what steps may come next.
Coordinating Care
When appropriate, case management can help improve communication and coordination among the different providers or organizations involved in your care.
Advocating for Your Needs
Navigating community systems can be complicated. Your case manager can help you understand your options and advocate for appropriate services.
Following Up
Case management isn't always a single referral or phone number. Your case manager can continue working with you as needs change or additional resources become necessary.
